How to prepare for a chat consultation before you apply for a medical certificate
This guide shows how to prepare for a chat consultation on DrCertificate, so your medical details are ready before you apply for a medical certificate. An independent registered doctor reviews every request after a digital chat consultation and decides whether to issue a certificate.
DrCertificate is for people aged 16 and over in 10 South East Asian countries, including Singapore, Malaysia and the Philippines.
The short answer on preparing for a chat consultation
To prepare for a chat consultation on DrCertificate, have your full name as written on your ID, the date your symptoms started, an honest description of your symptoms and the dates or purpose your organisation needs. An independent doctor reads these details and decides whether to issue a certificate. The service is for non emergency situations only.

What should you know before your chat consultation?
Your name must match your ID
Doctors decline requests when the name on the form does not match the ID you share.
Your details must be consistent
Symptoms, start dates and requested dates should tell one consistent story.
The doctor makes the decision
An independent MBBS, MD or MS doctor decides whether a certificate is issued and which dates it covers.
Check acceptance before you pay
Confirm that your employer, school or airline accepts online medical notes first, because payments are non refundable.
What is a chat consultation for a medical certificate?
A chat consultation is a written exchange in which an independent doctor reviews the medical details you submit on the DrCertificate form and decides whether a medical certificate is appropriate. There is no video call and no clinic visit.
The certificate that may follow states what the doctor concluded from your details: that you were unfit for work or study, that you should work from home, that you were unfit to travel, or that you are medically fit for a stated purpose. A certificate is not a treatment plan and not a prescription.
Picking the right option matters.
The DrCertificate form offers four choices. Sick Leave covers absence from work or university and general medical notes. Work From Home covers advice to work remotely. Unfit to Travel covers a trip you cannot take. Medical Fitness covers fit to work, fit to fly, sports fitness and good health certificates. People often confuse an unfit to travel note with a fit to fly note, yet the two say opposite things.
When do you need to prepare medical details for a chat consultation?
You need your medical details ready any time an organisation asks for a doctor's note and you plan to request one through a chat consultation. The doctor sees only what you write, so your preparation decides how smoothly the review goes.
The usual situations are easy to spot. An employee calls in sick and HR asks for a sick leave note. A university student misses a class or an exam and the faculty office wants proof. A new hire or student needs a fitness certificate before starting, and a traveller has to show an airline that they are fit to fly or explain why they cannot travel.
Each organisation has its own reason for asking.
HR teams use sick leave notes to record paid absence under company policy and local labour rules. Schools use them to excuse missed work. Employers and sports clubs ask for fitness certificates to confirm a person can take on a role or activity safely. Airlines and travel providers may ask for a fit to fly or unfit to travel note when health affects a booking.
Knowing which of these applies to you tells you which form option to pick and which dates to give.
Preparing for a chat consultation at a glance
This table lists what to prepare for a chat consultation and why the doctor needs each item. The number of days or the fitness outcome is always the doctor's decision, made case by case.
| What to prepare | Why the doctor needs it | What to keep in mind |
|---|---|---|
| Full name and ID | Confirms who the certificate is for | Use the same spelling as your ID |
| Date your symptoms started | Sets the period the doctor considers | Give the real date, not the date you wish to cover |
| Description of symptoms | Lets the doctor judge whether a certificate fits | Use plain words that match your dates |
| Purpose and recipient | Matches the certificate type to its use | Say whether it is for work, study, travel or fitness |
| Days requested | The doctor decides the number of days case by case | Sick leave and work from home notes usually go up to 7 days for generic symptoms |
| Fitness purpose | The doctor decides the fitness outcome case by case | Fitness notes are generally valid for 30 days |
| Severe or urgent symptoms | A written chat cannot replace an examination | Visit a clinic or emergency department in person |
Who reviews your details and issues the certificate?
Independent doctors with MBBS, MD or MS qualifications review your details and issue any certificate that follows a chat consultation. DrCertificate, operated by Health Venture OU, is the platform that connects you with those doctors. DrCertificate does not diagnose, treat or prescribe.
Every doctor on the platform is registered with a medical council.
The roles stay separate. You supply honest information. The platform runs the online form, takes payment through Stripe and sends approved certificates by email, while only the doctor decides whether a certificate is issued, which dates it covers and what purpose it states.
That split explains why preparation sits with you. A doctor working from a written chat cannot examine you in person, so the decision rests on the details you give.
How do you prepare for a chat consultation, step by step?
Preparing for a chat consultation takes seven short steps, from gathering your details to checking the PDF that arrives by email. The doctor reviews everything in a digital chat. There are no video or in person consultations.

Step 1: Gather your details
Have your ID, the date your symptoms began, a plain description of how you feel and the dates or purpose your organisation needs.
Step 2: Check what the receiving organisation accepts
Ask HR, the school office or the airline whether it accepts online medical notes and whether it needs a set format.
Step 3: Choose the right form option
Pick Sick Leave for work, university or a general note. Pick Work From Home for remote work, Unfit to Travel for a trip you cannot take, or Medical Fitness for fit to work, fit to fly or sports fitness.
Step 4: Give accurate details when you fill in the form
Enter your name as it reads on your ID and describe symptoms that match the dates you give. Mismatched names or inconsistent details lead doctors to decline.
Step 5: Pay through Stripe
Payment is taken through Stripe when you submit. Payments are non refundable, so check the option and your details before paying.
Step 6: Answer the doctor in the chat
An independent doctor reviews your request in a digital chat, may ask follow-up questions and decides whether to issue a certificate and for which dates.
Step 7: Receive and check your PDF
Approved certificates arrive by email as a PDF. Check the name, dates and purpose before you send it on.
You'll continue on the secure DrCertificate application form.
What should your certificate include, and what should you check?
A medical certificate usually shows your full name and the dates it covers, along with its purpose and the issuing doctor's details. Run the checklist below before you submit it to anyone.
Ask for your organisation's format first. A school or employer may want its own form or a stated field, and that is easier to plan for before you apply than after.
A certificate cannot be corrected once issued, unless the error came from the medical team. Mistakes you typed on the form will carry through to the PDF, so full preparation saves you a second paid request.
Name matches your ID
The spelling on the certificate is the same as on your ID.
Dates match what you told HR or school
The covered dates line up with the absence or purpose you reported.
Purpose fits the request
The certificate type is the one your organisation asked for.
Doctor details are present
The issuing doctor's details are shown on the certificate.
Will your employer or school accept a certificate from a chat consultation?
Your employer, school or airline decides whether to accept a certificate from a chat consultation. DrCertificate and the doctor do not control that decision, so check the receiving organisation's rules before you apply.
Questions usually come up for plain reasons. The certificate type does not match the request, for example a sick leave note sent when HR wanted a fitness certificate. A detail the organisation expects is missing, or the dates fall outside company policy. Sometimes the organisation does not take online medical notes at all.
If HR or a school office questions your certificate, ask which detail is the problem and point them to the doctor details shown on it. Every doctor on the platform is registered with a medical council. If the issue is the wrong certificate type, a new request with the correct option is the fix, and that is a separate paid request. If you believe the medical team made an error, write to care@drcertificate.com.
Checking first is the surest protection.
When is a chat consultation not the right choice?
A chat consultation is not the right choice when you need urgent care, when your situation falls outside the service's rules, or when you need a certificate DrCertificate does not offer. The doctor may also decline any request.
Seek in person or emergency care straight away for chest pain, trouble breathing, sudden weakness or heavy bleeding. The service is for suitable non emergency situations only.
A longer or more serious illness is better assessed at a clinic, where a doctor can examine you.
- Not for submission in India Certificates from DrCertificate are not for submission in India.
- Not for legal or insurance use Certificates are not for legal purposes and must not be used for insurance.
- Applicants must be 16 or older Anyone under 16 cannot apply.
- Recovery and caretaker certificates are not offered Neither type is available on the form.
Preparing for a chat consultation in Singapore, Malaysia and the rest of South East Asia
DrCertificate serves 10 South East Asian countries. The list covers Singapore, Malaysia, Indonesia and Brunei. The Philippines, Thailand and Vietnam are included too. Cambodia, Laos and Myanmar complete the list.
Rules on when a doctor's note is needed and what it must show change from country to country, and company policy can add its own conditions. In most South East Asian countries, sick leave and work from home notes go up to 7 days for generic symptoms, and fitness notes are generally valid for 30 days. The doctor still decides each case.
Check the official rules with your country's labour or health ministry, for example Singapore's Ministry of Manpower, Malaysia's Department of Labour or the Philippines' Department of Labor and Employment.
Then confirm with your employer, school or airline before you apply.

Frequently asked questions about preparing for a chat consultation
Whether a certificate covers days before your consultation is the doctor's decision, not yours. Give the real date your symptoms started and do not change it to fit a deadline. The doctor sets the dates from your details, and your employer or school may have its own rule about notes that cover past days.
In most South East Asian countries, sick leave and work from home notes go up to 7 days for generic symptoms. The doctor decides the number of days case by case, based on what you report. A longer or more serious illness is better assessed in person at a clinic.
You do not have a phone, video or face to face conversation with the doctor. The doctor reviews your form in a digital chat and may message you with questions. Answer promptly and accurately, because the doctor relies only on your written details to decide whether to issue a certificate.
If the doctor declines, no certificate is issued and the payment is not refunded. Doctors decline when details are wrong, for example a name that does not match the ID, or when the symptoms and other details are inconsistent. Checking your details before you pay is the best way to avoid a decline.
Have your ID, the date your symptoms started, a short honest description of your symptoms and the dates or purpose your organisation needs. For a fitness certificate, know what it is for, for example a new job, a sports event or a flight. Confirm the receiving organisation accepts online notes too.
A certificate cannot be corrected once issued, unless the mistake came from the medical team. Errors you made on the form stay on the certificate, so check your name, dates and option carefully before you pay. If you think the medical team made the error, email care@drcertificate.com.
