To get a medical fitness certificate online with DrCertificate, fill in the online form, choose the Medical Fitness Certificate option and pay through Stripe. An independent certified doctor reviews your details by chat and, if satisfied, emails the certificate as a PDF, usually in about 30 minutes. Issuance depends on the doctor's judgement.
About this certificate
A medical fitness certificate is an official document issued by a qualified doctor. It states that you have been medically assessed and are considered fit for a particular activity or requirement.
Organisations may ask for one to make sure you can safely perform certain duties, take part in activities or meet health-related eligibility criteria.

These details help employers, universities and organisations confirm the certificate followed a proper medical evaluation.
Avoid fake certificates. Blank templates, edited sample certificates and fake certificate generators may lead to rejection. A genuine certificate is issued only after a real health assessment by a qualified doctor.

Every request is assessed individually. Issuing a certificate, its content and its duration are at the reviewing doctor's discretion. Acceptance depends on the organisation you submit it to.
Employers, schools and airlines usually expect a medical fitness certificate to show your full name, a plain fitness statement, a recent date and the doctor's name and registration. Beyond that, each recipient sets its own rules, and those rules differ by employer, institution, airline and country.
Formal workplace exams follow their own law. In the Philippines, the Omnibus Rules that carry out the Labor Code say the physician engaged by an employer shall conduct a free pre employment medical examination for the proper selection and placement of workers, plus a free annual physical examination.
Singapore's Ministry of Manpower says employers must send employees exposed to named occupational hazards for pre placement and regular medical examinations. The same ministry says the migrant worker medical examination screens for 4 types of infectious diseases and checks whether the worker is fit to work. An online note does not replace any of those exams.
Before you apply, ask the recipient three questions.
Does it accept online medical notes? Does it have its own form or format? How recent must the certificate be?
A DrCertificate medical fitness certificate includes your personal details, the date of issue, the doctor's fitness statement and the issuing doctor's details. Recipients can read the layout at a glance.
The doctor's section shows the doctor's name, medical qualification and medical council registration details, which a recipient can use to check the doctor.
The dates show when the doctor reviewed your request and, where relevant, the period the statement covers. In most South East Asian countries, fitness notes are generally valid for 30 days.
Write the purpose of the certificate in the form if you want the statement to mention it.

How it works
Add your details and the reason for your request. You may be asked to share vitals or other measurements through a secure link sent by email.
The doctor assesses the information you provide and decides whether a certificate is medically appropriate. They may decline a request.
Approved certificates are sent electronically to the email address you provide.
You'll continue on the secure DrCertificate application form.
Choose the Medical Fitness Certificate option, which covers employee fitness, student fitness and good health certificates. The same option is used for sports fitness, fit to fly and fit to work requests.
In the details box, write who will receive the certificate and what it is for, for example a new job, a university course or a flight date.
If no option fits, email care@drcertificate.com before you pay.
The doctor will ask about your current health, past illnesses and the date and purpose of the certificate. Clear answers help the doctor decide faster.
Expect questions on ongoing conditions, recent illness or injury, regular medicines and any symptoms during exercise or daily activity. Your name and ID details must match each other, because a mismatch is one of the reasons doctors decline. If you have recent reports from a clinic, mention them in the form.
Keep your phone or email close by.
The doctor may send follow-up questions in the chat before making a decision.
A DrCertificate medical fitness certificate cannot be used for submission in India, for legal purposes or for insurance. Applicants must be at least 16 years old.
The service is for suitable non emergency situations only. If you have chest pain, trouble breathing, a severe injury or any urgent symptom, go to your nearest emergency department or call local emergency services straight away.
The recipient decides whether to accept the certificate, so check with them first.
If the doctor cannot issue your medical fitness certificate, you receive no certificate and the payment is not refunded. Doctors decline about 2% of requests.
The usual reasons are wrong details, like a name that does not match the ID, or symptoms and answers that do not line up. A doctor will also decline when your situation needs an in person examination or tests, for example an employer's pre employment exam or a government medical screening. In that case, visit a clinic or the doctor your recipient names.
Payments are non refundable. The one exception is a verified technical fault reported within 24 hours.

A medical fitness certificate costs 25 SGD in Singapore, 399 pesos in the Philippines and 30 MYR in Malaysia. Prices in other countries depend on the country.

| Country | Price | What the fee covers |
|---|---|---|
| Singapore | 25 SGD | Doctor review, plus the PDF certificate if approved |
| Philippines | 399 pesos | Doctor review, plus the PDF certificate if approved |
| Malaysia | 30 MYR | Doctor review, plus the PDF certificate if approved |
You'll continue on the secure DrCertificate application form.
Your employer, school or airline will accept a medical fitness certificate only if its own policy allows online medical notes. DrCertificate cannot decide that for the recipient, so ask before you apply.
All doctors on the platform are registered with a medical council. A recipient who wants to verify the certificate can check the doctor's name and registration details against that council's register.
If the recipient requires its own form, ask whether it will take the DrCertificate format instead. If the answer is no, visit a clinic in person.
A medical fitness certificate says you are fit, while a sick leave certificate says you are unwell and need rest. Picking the wrong one wastes your fee, so match the certificate to what the recipient asked for.
