If the doctor decides you are fit to run, the certificate is issued as a PDF and sent to your email.

What is a marathon fitness certificate?
A marathon fitness certificate is a medical note in which a certified doctor states that you are fit to take part in a long distance running event, based on the health details reviewed. It records the doctor's opinion at the time of the review.
The certificate does not diagnose anything. It does not treat a condition, prescribe medicine or promise that race day will go without problems, and it does not override a race organiser's own entry rules.
You will see other names for the same document. Organisers and clubs often say medical certificate for a marathon, running fitness certificate, race medical clearance or sports fitness certificate.
All of them describe one idea: a doctor saying you are fit to run.
When might you need a marathon fitness certificate?
You might need a marathon fitness certificate when a race organiser, club or employer asks for written proof that a doctor has cleared you to run. The request usually comes at registration or before you collect your race bib.
Common situations include entering a full or half marathon, signing up for an ultra or trail race, joining a running club, or representing your company or university in a team run. Races abroad can ask too, and the rules are set by the event, not by your home country.
The person asking is usually the race organiser. Running clubs, school sports offices and employers that sponsor corporate teams ask as well.
A different certificate fits better in other cases. If you are unwell and need time away from work or study, a sick leave certificate is the right choice. If illness stops you from travelling to a race, look at an unfit to travel certificate.
What do race organisers, employers or schools usually expect from a marathon fitness certificate?
Race organisers usually expect a short note signed by a doctor that names you, states you are fit to take part, and is dated within a set window before the event. Each organiser sets its own window and its own wording.
International guidance shapes these rules. The World Athletics Competition Medical Guidelines say runners should be warned about cardiac risks and advised to seek medical clearance from their personal physician, above all if they fall into higher risk groups.
The TCS London Marathon medical advice points the same way. It tells runners to talk to a doctor before training starts, above all with a family history of heart problems, high blood pressure or diabetes, or with symptoms like chest pain, breathlessness, palpitations or loss of consciousness.
Rules differ by race, club, employer and country.
Before you apply, confirm three things with the recipient. Ask whether they accept online medical notes, whether they need their own form or wording, and how recent the certificate must be on race day. Check too that the name on your race entry matches your ID.
What does a marathon fitness certificate include?
A marathon fitness certificate from DrCertificate shows your personal details, the date of issue and the doctor's statement on your fitness to take part in running events.
The doctor's name, qualification and registration details appear on the certificate, so a race organiser can see who signed it. Every doctor on the platform holds an MBBS, MD or MS and is registered with a medical council.
Fitness notes are generally valid for 30 days in most South East Asian countries. Your race may set a different window, and the organiser's rule is the one that counts.
If your race needs set wording, mention it in the form.
How do you get a marathon fitness certificate in 4 steps?
You get a marathon fitness certificate from DrCertificate in four online steps. There are no video calls and no in person visits. The doctor works from your form and a chat.
Fitness requests are reviewed from 9 am to 9 pm IST Monday to Saturday and 10 am to 8 pm IST on Sunday. A typical review takes about 30 minutes, depending on doctor availability and how clear your answers are.

Step 1: Fill in the online form
Choose the Medical Fitness Certificate option, enter your details as they appear on your ID, and say the certificate is for a marathon, with the race date and distance.
Step 2: Pay through Stripe
Pay the fee for your country at the Stripe checkout. Payment is non refundable.
Step 3: Chat with the doctor
An independent certified doctor reads your request, asks any follow up questions by chat, and decides whether you are fit to run.
Step 4: Receive your PDF by email
If the doctor approves, the certificate arrives in your inbox as a PDF, ready to send to your race organiser.
You'll continue on the secure DrCertificate application form.
Which option should you choose on the form for a marathon fitness certificate?
Choose the Medical Fitness Certificate option for a marathon fitness certificate. That option covers sports, employee, student and good health fitness notes.
In the form, write that you need clearance to run a marathon and add the event name, date and distance.
Not sure the option fits? Email care@drcertificate.com before you pay.
What will the doctor ask you for a marathon fitness certificate?
The doctor will ask about your general health, any heart or blood pressure problems, and how your training is going. Expect questions about the race date and distance as well.
Be ready to share details on diabetes, asthma, recent illness or injury, and the medicines you take. Mention any family history of heart problems too, along with chest pain, breathlessness, palpitations or fainting, even if it happened once.
Recent health check results or the organiser's own form can help, so keep them close.
The doctor may ask follow up questions in the chat before deciding. Clear, accurate answers make that decision easier.
Where can a marathon fitness certificate not be used?
A marathon fitness certificate from DrCertificate cannot be submitted in India, used for legal purposes or used for insurance. Applicants must be at least 16 years old.
The service is for non emergency situations only. If you have chest pain, trouble breathing, a collapse or any urgent symptom, contact local emergency services or go to the nearest hospital now.
Whether the certificate is accepted is up to the race organiser. Check with them before you pay.
What happens if the doctor cannot issue your marathon fitness certificate?
If the doctor cannot issue your marathon fitness certificate, you receive no certificate and the payment is not refunded. Doctors decline about 2% of requests.
A decline usually happens when details are wrong, for example when the name and ID do not match, or when the health details shared are inconsistent. A doctor may also decline when your answers point to a need for a physical examination, an ECG or other tests that cannot be done by chat. In that case, see a doctor in person before race day.
The only refund exception is a verified technical fault reported within 24 hours.
Questions go to care@drcertificate.com.

Who issues your marathon fitness certificate?
Will my race organiser, employer or school accept a marathon fitness certificate?
Acceptance depends on the race organiser, employer, school or club that receives your marathon fitness certificate. Each one sets its own policy, and DrCertificate cannot decide it for them.
A recipient who wants to verify the certificate can use the doctor's name and registration details printed on it to check with the medical council where the doctor is registered.
What if the race has its own medical form?
Ask the organiser whether a separate certificate is enough. If they insist on their own form signed in person, an online certificate is not the right choice for that event.
Marathon fitness certificate vs related certificates: which do you need?
A marathon fitness certificate states you are fit to run a race. Its closest relative, the medical fitness certificate, states you are in good health for work or study. Both use the Medical Fitness Certificate option on the form.
Sick leave and unfit to travel notes do the opposite job. They record that you need rest or should not travel.
| Certificate | What it states | Who usually receives it | Form option to choose |
|---|---|---|---|
| Marathon fitness certificate | You are fit to take part in a running event | Race organiser or running club | Medical Fitness Certificate |
| Medical fitness certificate | You are in good health for work or study | Employer, school or agency | Medical Fitness Certificate |
| Fit to fly certificate | You are fit to travel by air | Airline | Medical Fitness Certificate |
| Sick leave certificate | You need time off because of illness | Employer, school or university | Sick Leave |
| Unfit to travel certificate | You should not travel for health reasons | Airline or event organiser | Unfit to Travel |

Frequently asked questions about the marathon fitness certificate
In most South East Asian countries, a fitness note is generally valid for 30 days from issue. Your race organiser may set its own rule, for example a certificate dated within a set number of weeks before race day. Check the organiser's window and apply so the issue date falls inside it.
No, you should not ask for a backdated fitness certificate. A marathon fitness certificate records a doctor's opinion of your fitness at the time of the chat review, so the date reflects when the doctor assessed you. Plan your application so the issue date suits the organiser's deadline.
Your marathon fitness certificate arrives by email as a PDF if the doctor approves your request. A typical review takes about 30 minutes during fitness review hours. Check your spam folder if it does not appear, and email care@drcertificate.com if nothing arrives.
No, a certificate cannot be corrected once issued, unless the error came from the medical team. Enter your name as shown on your ID and race entry, and double check every date before you pay, because a mistake on your side means a new paid request.
